I have tried to submit my form on 3 separate devices now and all have showed that the form was successfully completed after I finished filling it out but when I go to pull up the forms again they are nowhere to be found. Please advise ASAP

I took a screen shot of the links you sent me, both go to an error page.
Above is a link to the consultation forms. Yesterday we submitted 2 of these forms and neither of them showed up in our inbox and we completely lost that information.

When using the links provided by my colleague, I get the same error. Please try this one instead:

When I cloned your form and made test submissions they come through without issue. May I please make a test submission on your live form to see if I can replicate the error that way?

I just submitted a form as a test and it seemed to go through without issue now. You can try yourself if you would like. To avoid this from happening in the future is there anything we can do?

We are glad that you were able to successfully make a submission on your form without any issue. When opening the form make sure that you are using the form URL to avoid any issue in opening or submitting it.
