Dear JotForm,
My form is working fine as per my requirement, except for 1 issue. The chekcbox values on my form are not being sent on our registered email. I can see the values being stored in the database, but they are not visible on the email.
I have deleted them and added several times, but surprisingly it works once a while and mostly just blank data.

Hi Jeric,
Sam_G replied to my earlier request and I tried the solution as per her instructions. I kept the previous form variable and added a value from "Form Fields" in Email settings. It seems to be working now. You can view the changes from the form:

I've clone your form and I was able to resolve the issue by deleting the existing email notifications setting and adding a new one. Please screencast below:
