    Embed Code makes to appear part of my web page instead of the form

    If I place the code below on a page it adds the header and footer of the theme.

    Hi,

    I can see what you mean.

    Please try re-embedding your form using the iframe code at this time instead of the Embed one. Here is how to get the iframe code of the form: Getting the form iframce code

    Probably this is caused by a script conflict between your form and your web page, so using the iframe code will protect your form script as it is into an iframe container.
