Import Data from MYSQL Databas

Is it possible to fetch my form with information stored in MYSQL database. If it is possible how is it done?

Unfortunately, we do not have this function in our form builder. The only way I could think that would make this possible is by creating a PHP script that would fetch the information from your MySQL Database to the form, using the source codes: How-to-get-your-Form-Source-Code. We offer a workaround, but this one is a bit difficult so I'm afraid that I can't help you with this. You may consider hiring someone to do it for you. You can go to oDesk.com or Elance.com for freelance programmers that specialize of doing this stuff.
