Audit form with recommendations

    Hi there,

    We have a compliance audit form with about 70 questions that we ask our clients. It is currently in Word but I'd like to transfer it into a proper form. So I'm wondering whether JotForm has functionality where I can create a form with the following fields:

    1. A field for the actual question and space below each question for me to type in/record their answers
    2. and then a space to the right of that where I can colour a box (eg. Red or green) based on whether their answer indicates they are compliant or non-compliant (risk level)
    3. and then a final space to note our recommendations for action

    Below photo is an example of how it would look on the page.

    I'm also interested to know whether JotForm has functionality where I can have various recommendations already in the recommendations column and based on their answer, the form will select the most appropriate recommendation to keep and which to delete?

    Thanks for your advice on this. I'm thinking of upgrading my subscription if this sort of thing can be done. Cheers
