Hello Sam,
It seems you are using custom URL. The above form URL will not work because you are using date in the URL which has separator "/". Technically "/" will check for a child folder in the web server.
Please avoid the "/", "'", ";" and any other special characters from the custom URL and that should solve your problem. Please be noted that "_" and "-" will work just fine in your form custom URL.

Just to add to my colleague's advice, the apostrophe in St_Paul's will also cause a problem.

Have you tried creating another form using the same procedure and account but and did not work as well?
