I am wanting to purchase jot form to set up a hippa compliant way to enter and report out covid 19 rapid antigen test results.
I see that I can purchase a silver membership which does have hippa compliance.
I am wanting to be able to email and print out results and set up a fillable form form a ipad or smart phone.

Hi there,
May we know if the form is solely for test results or also for people who will undergo the Covid testing? In yes, once the test related form is established, you can add a button on the submission table that you will click to send an email to the enduser to let them know of the results.
1.) Create a hidden test results Single Choice field;
2.) Create two autoresponder emails, one for when the result is Positive, and another one when Negative.
3.) Create two Change Email Recipient conditional logic so the autoresponders emails don't get sent automatically.
4.) Once all above is set, create a Positive and Negative buttons on the submission table which you will click when the result is already available.
So, users fill out the covid test form, once the results are available, you will click the corresponding button to the result on the submission table. Feel free to check/clone my demo form.

Hello Deanna,
Yes, you can generate test results. However, you need to disable the PHI fields to show them in an email field.
