Cannot delete an old entry from our enterprise HIPAA enabled form

We have an entry that is unable to be deleted or moved to archive so that it is not in our active submissions list for one of our questionnaires. The entry looks like it's garbage information, but it's from over 5 months ago and is meaningless to us as it has no identifiable answers or information we can use. We try to delete it over and over again, but it keeps showing up, and I tried to move it to archive, but it also keeps coming back into our submissions table.

Please help me fix this asap!
