Conditions disappeared overnight & revision history missing completely

My form (called Clone of VBS 2013 Registration) was working fine as of Tuesday, April 15th at 11 p.m. Central Standard time (after putting in 3 hours of work and about 20 conditions).  The next day all those new conditions that made it work were gone and the revision history doesn’t even show I was working on it late that Tuesday night.   Please find the revision history and restore to around 11:00 April 15th version.  The conditions missing were all related to the fields asking what grade a child is in.  If a child is in 3rd grade or above the fields related to the sign-out details shouldn't be visible.   Each time I added a conditoin, I verified the form was working and saved it.  

I did contact Jotform aobut this last week, but checked the "keep this question private" box and haven't gotten any response.  I'm on a May 1 deadline for this form.  Please help!

I checked your form and there are present conditional logic settings to it. I check your form direct URL: http://www.jotform.us/form/40915220196147 and they seems to be working as expected. Do you managed to resolved this?
