Why are errors showing up in my form?

Why are errors showing up in my form? I’ve tried everything i can to fix it. It looks like it’s due to me adding in a box where people can put in notes. https://form.jotform.com/220114010108027

I guess what i mean here is,

see where it says error in this page

Anytime i click on anything it pops up, and when i scroll down and see what the problem is - the box where i allow viewers to input any “notes” it is highlighted there

I was unable to recreate this issue on Chrome. Could you confirm if you have embedded this form somewhere else or where your user was directed from another website to Jotform? 

If your form is embedded somewhere else or was directed from another website, please share with us the URL. It may be due to the embedded method conflicting with your website script, or due to the specific application used to open the form, causing the error.
