We have a request for different dates in our form and some will work and some will not. How do we figure out why? How do we fix this?

Thank you for reaching out to us.
I cloned your form and I was able to replicate the issue. Some of the date fields are automatically changing the date format to MM/DD/YYYY despite setting it as MM-DD-YYYY which is causing the error.
I will escalate this to our back-end team for further review and correction. We cannot provide an ETA as to when this will be fixed but we'll be sure to notify you as soon as we receive any word from them.
While our developers are looking into this, what you can do is disable Lite Mode in the Date Picker field as a workaround.
This will divide the Date into three separate fields. Since this will not use any date format, the error should not occur in this setting.
