Just upgraded to Silver, cannot view/access HIPPA form. HELP!!!!

    Are you referring to the form 'Covid 19 Consent Forms'? If yes, this form is currently disabled on your account which is why you cannot open it. Please enable the form by going to the form builder and then settings under form status change it to enable.
