my form is disabled please help

    I do not know why my form is disabled
    my Blog in Arabic language
    that happened before because of my language
    i use the form in my blog in which the one who fill the form put all the information about his property and a way to contact him like his phone
    I did not do anything wrong 
    this is my blog
    Please help me
