    Hello Natasha,

    I checked the related form and I noticed that the form is disabled. To enable it again, the form owner can follow the steps below: 

    1. Open the Form Builder and click on the Settings.
    2. Change the Form Status to Enabled.

    After the form is enabled again, it will be ready to be used.
