Hello,
I created a new form, and while the questions that our clients are responding to are showing up both in the form and in the data submission section in the Jotform page, they are not showing up in the email response I get directly to my inbox, 3/5 of the questions present as "type a question" rather than the questions that I created when I made the form. I have attached a screenshot to show you.
I tried re-making the form from scratch, but when I tested, all of the questions say "type a question", so it got worse.

I apologize for the inconvenience. I cloned your form and could see the issue as well. However, when making a new Notification email, the issue seems to resolve.
Please give it a try and let me know if you run into further issues.
