Required field not registering

When I submit the form, it brings me to a screen that says the required fields are all required, eventhough I've inputted them. Where in the different code files should I go to fix this issue?

The field names are different from your standalone form when I checked the embedded form. Unfortunately, changing the field names will not let the form to capture the data and it would trigger that error. Please retake again the form source code and replace the existing codes on your page. Please do not make any changes on the field names this time, otherwise it will cause this problem.
