I can't log in, it keeps telling me that my account has been suspended

We apologize for the inconvenience. I reviewed your account/username coachcookie and I found it was suspended by the anti Phishing system due to the form that violates the Terms of Use.
You have form in your account that collect cash payments using cash apps and the like using the email.
Example:
This is not allowed. If you need to collect payments using the form, you can use payment integrations available.
Due to our PCI certification, we are required to deny the collection of payments using basic fields. We have other PCI-compliant ways to do this. All you need to do is choose any of the payment processors available in our platform in order to integrate in your forms.
If you only want to collect your customer's payment details and charge them at a later date, you can take advantage of a feature called "Payment Authorization". To learn more, kindly follow this guide on How to Enable Payment Authorization.
We can only request for consideration to reactivate your account and forms after you comply to the terms and agree that we can delete the illegal fields (i.e cash apps) on your form.

Please go ahead and delete the form I need access to my forms
My user name is coach cookie so the form payment/ Payment receipt please delete so I can't get my account back

Thank you for the quick updates. I have now deleted the illegal fields in the form and reactivated your account and forms. You can check the updated status when you login again using the username at https://www.jotform.com/login
