Need help in customizing my form

I'm new to JotForm, so if you're able to, I'd love your help with a few modfications for design and consistency to all three of my forms, which are launched from PutterPowerMedia.com, a GoDaddy-built and hosted website.

I'm trying to make the very best of this interim site before finding a suitable design partner to work with me on professional, custom websites for a trio of motorcycle-related businesses.

Here are the changes I'd like to make:

--Expand each form to 500 wide

--Have the header color match the light blue on my website

--Make all header type black

--Keep the small "mail" envelopes in the header design

--Make the forms' fields go nearly end-to-end horizontally (basically, make them wider, about 450)

--Center the type in each heading (not header)

Um, that's it. These descriptions amounted to a lot of words. Hopefully, the changes will take less time to make than it took me to write them.

To answer your question, I'd like to make the form's fields wider.

In addition to giving form users more space in each field to see more of the characters they're typing (rather than them being hidden), this would make the form look less lopsided (heavier on the left than right).

On another note, the changes made so far look great, but was looking for the header at top to be just a single box (PutterPowerMedia top, left; press release form, centered). A pair of envelopes on the right and the New Courier type in black would seal the deal!

A major bonus would be to have the header's background color match the light shade of blue seen on my website, PutterPowerMedia.com.

I hope all of this is clear.

Totally cool! The wider fields look great and the blue is you chose is a great match to the color on my site.

Last thing to fix in the press-release form:

--To build in additional contrast and offset the form's generous use of white type, please make all header (up top) and heading type black. This will look great on the blue backgrounds.

Then, if you're able to have my other two forms match the press-release form, I'd be over the moon.

I'll clone the press-release form to build my contact forms. Good suggestion.

Before doing so, if you could change the Courier New type in the green header at the top to black, instead of blue and white, that would be awesome.
