Can I create a form with jotform and have the input saved to a MySQL database located on my own server?

First of all, thank you for using JotForm.
In order to do that, you will need to alter the script/code of the form and house it in your own server. You might also want to try installing JotForm Application on your server. In this case, the database will be completely independent from JotForm's site. JotForm provides installation within 1-2 business days, but most of the time, in the same day. If you would like you can also install it yourself. All you have to do is to upload all files to your site and then complete the web based installer.
