I want just input boxes without text next to each one.

1)  How do I create this in my Jotform without having to add 3 text boxes.  I just want the 3 boxes on top of each other.

1. This is possible using this widget http://widgets.jotform.com/widget/narrative_fields

2. You can achieve this using Matrix Table under Survey Tools.
