    Dear JotForm,

    I'm a therapist and i use my website as a hub for information, referral, and assessment for current clients and new clients. Recently, the web dev team where my site is maintained stopped making JOT forms for us but did say we could build them on our own and submit them to them. I just did that with my client intake form--thanks to your amazing form builder that even a non-techie like me can use!--and theyve uploaded it and it's working great. I told them about how I found you guys via Google and that you would probably be a great referral source for them since they now have to tell their clients "go make your own JOT form and then send it in." I can tell you this--none of the therapists I work with had a clue as to what a JOT form was, but they all said "Oh, wow, great idea" when they saw mine and wanted their own. I've referred them to your site so they can make their own forms as needed, and just fyi it's TherapySites that hosts literally thousands of therapist websites, so you may want to connect with them and become the tool they refer us to in the future for JOT dev.

    Hello again and quick question--I'm going to be forwarding my JOT Intake form to a colleague who has been looking for something like this. Here's my question--you guys are familiar with the federal HIPPA guidelines on the encryption/handlding, etc. required for thetransfer of any patient's medical info via paper oronline, correct? I saw in your FAQ you do usedata encryption,and Ichanged the return emailfor my forms to by myemail so theforms are stored with me and not a third party,butcan I tell mycolleague this is a "HIPPA compliant" process? It will be the main thing she wants to know.
    Also, once we can verify this, I'd be happy to add that to my "feedback" since that's something people in my profession are always concerned about and rightly so.

    DO note that JotForm is no not HIPPA CompliantCertified in any way. Nevertheless the service can be used in a HIPPA compliant way. As long as the user takes the neccesary precautions to not allow the data to be exported through unsecured channels such as email alerts. 

    Please check the below details:

    Our servers already match all criteria since we already care a lot about the security. However, some features of our application are not HIPAA compliant so if you refrain from using those features, I think you should be fine.

    1. Always use SSL (https) version of JotForm site on your browser. Use "https://www.jotform.com" to login to your account, create your forms, look at your submissions and link to your forms.

    2. Edit emails on all forms to make sure no specific information is used on them. We send emails in plain text. So, they are not secure. Only use emails to get alerts to know there is a new submission. Once you receive an email alert, log into the secure JotForm site and then look at the user 

    3. If you use the Reports feature only do it with password protection. That will both ask for a password, and it will transfer all data over SSL.

    4. Same for uploads. They are not password protected.

    5. Logout immediate after you are done with the site.

    6. Regularly download submissions and then delete them.

    Data stored on our servers are not encrypted, but access to our servers is safeguarded. Data transmission from the person who submits their health information to our servers can be done in an encrypted manner, by using the forms securely.

    JotForm certainly complies with the technical safeguard section of the HIPAA security rule:

    Information systems housing PHI must be protected from intrusion. When information flows over open networks, some form of encryption must be utilized. If closed systems/networks are utilized, existing access controls are considered sufficient and encryption is optional.

    Well answered. So, in sum, if my clients's submissions of my JOT form are coming directly to me, there really is no HIPPAissue right? I'm not receivingor storing their intake info onyour site but rather mine. Am I correct in this?

    Hi Lynn,

    That's right. You can do it that way, that's really fine.

    If you are also embedding the form, please make sure to use its secure embed code. This ensures that data will be transmitted with 256 bit of encryption.

    Please check this visual guide on how to take your secure form embed code.

    I did check your web page and found that you are not using SSL version of your form and just using non secure form URL. Please check the screenshot below.

    Ok,after 5 attempts and almost blowing up my form :) I think I've got itright now--I just highlighted "Welcome to Therapy" and "edit link" and then pasted this in the box: https://secure.jotform.us/form/41085787297166
    This was after I went to your site, logged in and clicked the"Secure" button.
    How does thislook now?

    Hello Lynn,

    I did check your web page and found that you are still using your form's non secure embed code. Please check the screenshot below:

    Just checking the "Secure Form" checkbox will not make your form secure. Please be noted that you need to grab your SSL embed code to embed it in your web page. 

    Please replace the embed code you have added in your web page "WelcometoTherapy.en.html" with the following code:

    <script src="https://secure.jotform.us/jsform/41085787297166" type="text/javascript"></script>

    Hmm, I've done all the steps and when I click on the form on my site is says "secure" so I'll have the IT guys do it and I'm sure they'll be able to do it correctly :).

    I'm a therapist, and my industry has been paperless for awhile which is a great thing. But my practice requires I collect data from clients and keep it securely for a period of 7 years. I needed to update a jot form I'd had designed for my intake, but my IT person only works 2 days a week [now that was a great career choice!] so I Googled "jot form software" and found Jotform.com, liked how the site was set up with templates [hundreds of them], and started to play. Within about 5 minutes I had the intake form I wanted. In another 20 minutes, I had my dream intake form that I started sharing with colleagues who asked where I found this. For the next three days, I tweaked my new intake form, and I had an ongoing email exchange with some of the IT team at Jotform who answered all my questions with just the right about of detail for a non-tech person and even looked at my site to make sure I was able to make the changes we were discussing. Their tech support
    was beyond outstanding for a period of 3+ days til we got my form perfect. They should be commended for having both an excellent product and a similarly outstanding team of tech people backing up their product. Well done and thank you for helping me stay paperless!
