I would like to add an introduction to the questionnaire.

Hi - I am creating a new questionnaire and would like to format the sub header part of the header - how can I do that - our introduction has a couple of paragraphs. Thank you!

You should be able to create your introduction on your questionnaire by adding Paragraphs under the form elements.

Click on the Form Elements under the navigation panel on the left side of your form, and click on the Paragraph, this should help you create multiple paragraphs for your introduction.
