Lost form... not sure why

I have been working on a PO order form for the past couple of weeks.  I went in today to finalize the pricing options.  I got to the end of the pricing dialog boxes, clicked the 'Finish' button and waited... nothing happened.  I assumed that the changes I had made had been saved.  when I went back to edit form not only were the pricing options gone... the whold form below the header was gone.

I am counting an a form that I will be able to count on once implemented.  If I have to recreate the form I can... I just don't want to do this if there is a chance I will once again lose everything.

I have included the URL for the draft form I was working on... it was almost complete, not including the pricing information.

    Thanks you for your response.
