How do I get my form to show up properly on an android phone?

    Hi,

    I created this online service submission request form so that our clients can go online and submit their requests.  The form is showing up fine on the computer, iPad and iPhone, but for whatever reason it is not showing up well on my boss' Android phone. Please let me know how I can fix this since I also tried adjusting the width of the form and that has not helped.
