Simple form is really slow to load

I've got a simple form, but it is really slow to load. People could lose patience.

I checked your form http://www.jotform.us/form/41185834076155 but I was not able to encounter slow to load issue at this time. It loaded instantly when I browsed it.

It would help us check more about your case if you can provide us more details like.. is your form embedded on a website?

Share to us the URL of the website where the form was used so that we can also test.

Please also take note that, since these are web forms, the internet speed plays a factor on the speed performance. Of course faster internet will mean faster browsing.
