Terms and Conditions widget prevents form submission

Also when you click on the link (see attached image) and then run the form and get to the end and "submit" it just says "please wait" for ever and never submits the form.  What is going wrong??

Hi, clone your form, remove the  terms and conditions widgets that are located after the submit button, on the cloned form:

I cloned your form in my account, removed those fields, and it did work, test my form here. Let us know if you have more questions.
