How do I edit template form?

SPP Food Order Form.  I wanted to create a simple Food Order form for our website.  The template SPP Food Order Form in your order form templates looks like it would work, but I can't figure out how to edit the pics and text.  It is in Slavic, and I need it in English.  How do i edit it, and is it complicated to set up the form to take payments?
