I want to set up an online application that stores tha information in a database. I also want to be able to upload a file. I am not sure if jotform could do all of this. Please advise.

You can accomplish this by integrating the form with Google Drive, and Dropbox. Also, we have an alternative to FormAssembly HTTP Connector feature, you can send the form Post data to 3rd party script with us. Please take a look at the following article:
On regards of uploading a file in you form, do you mean that you want to have users uploading files on your form? Simply add a Upload Filed located under "Form Tools":
