I am not receiving the data of one of my matrix dynamique widgets, why?

Hi,

I have been using Jotform for a while and really like it.

However, in a form I have, called "Food Vendor Sign-up" I have a Matrix Dynamique widget that seems to be losing information all the time.

I am not sure what happens, as when I submit test forms, all goes well in the matrix dynamique, but then when people submit forms to me, I have something like what you see here in the attached picture.

Could you please suggest a fix for this?

Alternatively, is there any other type of widget that you could recommend for adressing a similar question that requires multiple fields?
