I've been reviewing your form: 41197476987373.
I do see that Notifier1 (which is the Email Alert set to send Emails to: email@example.com) does not have a Sender Email setup:
Please set a Sender Email on that field, such as firstname.lastname@example.org, to continue receiving your email alerts.
