my form does not show in preview, also how can i create a unique url for my form so i can point my visitors to it. do i have to have a website or can i just use your address plus my buisness name.

Thanks for asking. Every form has a unique URL but I think you are referring to its customized URL. Your customized URL is not in the correct format and this is the cause of your not being able to preview the form. You need to replace what is currently in the Custom URL text box with a string that contains neither spaces nor URLs. To put it another way, you can enter either a single word or multiple words separated by underscores or hyphens. (camelCasing is also acceptable.) However, URLs won't work.
After saving the change, you should be able to preview the form without any problem.
When I logged in to your account, clicked the form and then clicked the Preview button on the form, I got the same result you did. However, when I loaded the form into the builder and clicked the Preview button in the toolbar, the form was displayed.
The reason for this is that the former preview method displays the form at its customized address (if there is one) while the latter uses the default address.
Your second question was partly answered above. You don't need to have a website to to display your form. After you have corrected the problem above, the direct link to your form will be in the box indicated in the screenshot below:
Click it to select it, copy it and share it as you see fit (via email, for example).
