    Is the problem that the form cannot go forward to the second page?

    As you can see in the screenshot Next and Back button appear in the first page, which is not correct. That means that some script conflict is happening between your form script you the one of your webpage due to the embed method you are using.

    Upon inspecting your page source code I found that you are using the Embed code to add your form to your web page. So, to resolve the issue kidly re-embed your form using the iframe code, that will protect your form from script conflict. Here is how to get the iframe code: Getting the form iframe code
