Submissions are not being set to the designated email address. Apparently this issue has been happening for about 2 weeks. I verified with our IT folks that it wasn't an issue with the spam filter, etc. It isn't the email address as I've tested it outside of JotForm. Please advise. Thanks.

First of all, thank you for using JotForm.
I've tried replicating your issue on our end and it seems your form is working just fine. The emails are sent without any flaws. Nevertheless, we will look deeper into this. For the mean time, if you could provide us other information that may help us solve your issue, that would be great.

Hi, I changed the email address for the notification email to email@example.com instead of the email entered by the customer and completed the form using an email address outside of our network. I DID receive the submission. I went to another form that we have (http://ehrtools.com/setupforms/ehrfaxsetup.html) to change that email address to firstname.lastname@example.org but it was already set to that address. I tested that form and we never received it. I had IT check and they check the ORF Log Viewer and said it never even made it to us. I hope that helps. Thanks!

The form in the address http://ehrtools.com/setupforms/ehrfaxsetup.html has a little something that's not needed in the Recipient E-mail field of the form's notification email setup.
You must have copied an email address's link instead of the actual text. Simply removing the mailto: should fix your problem.
