Install the form i've created to my website

hey guys

i have a problem .. i've created a form.. and when i try to view it .. i get this error http://prntscr.com/3ggh83

and i'm asking if u could please install the form i've created to in my site including

a page that only show the results to my admins/moderators in my website 

i can give you my admin password . also i got option in my host that can creat a website page can be show only for admins/moderators
