Unfortunately I was not able to view your form in the link you provided, it just gave me an error message saying "You do not have permission to preview drafts."

Please make sure to follow the instructions of this guide to correctly embed your form to Wordpress: Adding forms to Wordpress

If the issue still persists, kindly share with us the link where your form is embedded so that we can further investigate.

    Thanks

Thanks for the providing the URL of your Wordpress site.

Upon inspecting your site source code I found that you used the Source Code of your form to embed it. So, it is most likely this is the cause of the issue, since your form script is exposed it may be getting in conflict with a script of your web page.

So, please try re-embedding your form to your web page, but at this time use the iframe code instead. Here is how to get the iframe code: Getting the form iframe code

Since the script is within an iframe it cannot get incoflict with other scripts, hence the form should work properly now.

Inform us if the issue still persists.

    Thanks
