    I am not exactly sure about what you want but base on your title you're looking for a dynamic form. Is this about dynamic fields which users can automatically add dynamically within your form? If yes, we have few dynamic field widgets that you might want to try.

    not rerally,

    i was looking for something like

    Name: Blah Blah

    Address: blah blah

    Do you agree

    yes or no

    The name and address bit is filled by us,

    the yes and no bit is a radio button.

    when we send an e-mail it has the name and address bit filled by us and the user has to only click on yes/ no.

    Can this be done?

    @salomeb,

    Yes this can be done you have to use prepopulation feature to make the value in Name: and Address populated then make the field as "Read Only" so it won't be editable.
