The page is intermittently directing me to another capture screen following submission. When this happens, the form content that was entered is not transferred to my email account; no record of the submission is listed in the Jotform records. This problem started after I fixed the tabs that were automatically generated after I installed the "City, State"

When a second captcha appears the means there is a conflict between your site's jquery scripts and the form embed version. Please replace your current embed codes with the iFrame one.
