From my understanding the forms in question are:

It appears that the email address used on both forms: firstname.lastname@example.org. I have tested your form using my test email and they appear to send the email alert properly:
In which case, please make sure that the email alerts are notbeing redirected to your Junk/SPAM folder on your email account.
If you are accessing your email on a standalone email client such as Outlook, please try accessing your email account through its webaccess as some standalone clients may not download all email that is being filtered to your online junk folder.
Additionally do contact your email service provider to verify our emails are not being rejected.

Thank you for the reply. It appears there are two links for each of these. Could you please advise us as to where the below links are emailed? We have clients that have filled these in and would like to recover them.

Your email address is not in our bounce list and your Email Alerts configuration is also ok. So, you should receive email.
I have sent a test email to your email@example.com address.
