The link for my form doesn't seem to work

    Here is what I am putting into my email but ot doesn't seem to show up as a link.

    The details that you're putting is a script embed code that's used for embedding the form on web pages.  If you wish to use the URL as a link, you should use this instead:

    http://form.jotform.us/form/41255936016150
