Jotform Account Disabled

    I received an email today that 
    "You have currently reached the limits of your account. Your forms are disabled now."

    I upgraded this account to Jotform Premium Monthly on May 5. 
    Order ID: INT140506-8423-37500

     Please check your records and advise what I should do next.

    Hello Richard,

    Upon checking your payment details, I found that your have not mentioned your account username while making the subscription payment. That is the reason why your account was not upgraded automatically.

    I have taken care of your account problem. Your account is now upgraded to premium monthly subscription.
