Hi,
If you will only be using the direct URL of the form (example: http://www.jotform.com/41294102527145 ) there is NO means to add a login/password field protection into the form.
Having combination of components like username/e-mail/password/login field into the form is also not allowed -- as it can be used for phishing purposes.

BUT if you will embed the form into your website, you can instead use your website's login page to add access protection to the form.
