What is the email address you used in your account? I only see guest accounts associated with this email "ashley********@charterone*****.com". Did you already signup an account with Jotform? If not, please head here to create an account http://www.jotform.com/signup. Otherwise, if you already have an account, you can reset your password by clicking the forgot password link while on the login page http://www.jotform.com/login.

Yes I have an account and a form set up under my email address. Whenever I click the reset password I do not get an email or anyway to reset it. I have done this several times.

Sorry but there's no account created under your email address "ashley********@charterone******.com". All I can see is a guest account which means you have created forms with us without logging in. You have to register an account first using the same email address you used on your form so the form will be automatically carried over to your registered account or we can manually move your form to your account if you already have registered an account with Jotform.
