Hi Iain ,
I inspected your form and found out that you have not added 'mp3' in the allowable extensions for your upload field.
To change this:
Edit your form, click on the Extensions button in the toolbar, then add 'mp3' in the list (preceded by a comma). Or you may remove all listed extensions and just enter 'mp3' if you only want to allow mp3 files to be uploaded.
Click OK then save your form.
Get back to us if it still does not help. Leave your form as it is so we can inspect what causes the problem, if it still persists. You may also open a new thread for questions that are unrelated to this issue. Thanks!

In addition to what my colleague NeilVicente has shared with you, also consider the maximum allowable size of the uploaded files.
And, as an additional safety blanket, include other case combinations of the file extension, i.e. Mp3, mP3, MP3, mp3.
Please let us know if you need further assistance. Thank you for using JotForm!
