Problems with forms

Employees are having issues with Captcha, and submit buttons.  Forms not allowing submissions.  I've had a number of issues lately.  I don't understand.  Please help.

Hi, I just tried your form, and it is not being submitted, it gets stuck on "please wait", however when cloning the form it works, try my cloned version of your form here. Please clone your form on your account, and let us know what happens.
