How to Use the form

I created a form but now how do I use it and collect the data? Will it allow me to upload information into a excel spreadsheet? 

I now have the form but how do i use it online?  Do you have a number I can tALK TO SOMEONE? i CAN BE REACHED AT 202-262-8840.

You have to embed the form to your live (which mean online) website in order to collect data using the form you created.
