Problem occur in Image Uploading wizard. Please refer attached screenshot.

I ran a test on a copy of your form and it seems the problem might have been temporary.
Please try again. You may want to delete the image field and drag in a new one:
Try logging out then back in of your Jotform account if the issue persists.
