Hi m3amer,

Your form was suspended due to phishing. One of your form (اهلا بكم في برنامج زيادة الذهب في ستاردول ) has a field for username and password and it violates the JotForm's Terms of Use. Kindly remove password fields on your forms to keep your account from being suspended.

I have re-enabled your account. Kindly remove password field to keep your account from suspension.

Your account is a Free account and therefore, renewal is not necessary. Should you wish to upgrade you account again, and once username and passwords fields are removed from all your forms, kindly go to http://jotform.com/myaccount/billing and click switch plan under the plan you wish to subscribe.
