Scheduling patients

    I'm a psychologist in private practice, and I'd like to design a simple appointment form, to be used in conjunction with Gmail (Google Apps).

    I'd like to invite patients to 45 minute appointments, and ideally, have these entered into Google Calendar. Is this possible?

    Once you are on the edit mode of your form, follow this steps to add a google calendar to your form:

    1. Add a free text (HTML) field in your form where you want to display the google calendar. You can find the Free Text (HTML) field under "Power Tools" tab.

    2. Grab the iFrame embed code of your google calendar. This guide should help you:  http://www.webdesignerdepot.com/2012/04/integrating-google-calendar-with-your-website/

    3. Add this iFrame embed code in your free text (HTML) :

    Thanks very much! Do I send this directly to invitees, or can I embed this
    in a Gmail?

    You can share your form to others via email. All you need to do is paste the Form URL on the E-Mail you're going to send them. This way they can click on the Form URL so that they can access the form.

    I can't find Power Tools below Survey Tools on my blank form; nor can I
    locate Edit Mode.

    Our apologies! The Free Text HTML field is moved up to the "Form Tools" section and currently labeled as "Text":

     

    I would also advise to register first for an account: http://jotform.com/signup. I checked your email address, but I can't find any account that is associated to it so I assumed that you don't have an account yet to Jotform.

    Once you have an account and logged in, you have to access My Forms page and create the form:
