I noticed that I no longer receive emails from my form. Previously it had been working perfectly.
I asked my webmaster to look into it and she was unable to login using my username: johnturk & password. She attempted to initiate a password reset and I never received any emails from that either.
1. Why did my form stop working?
2. Did my account get accidentally deleted?

I checked your account johnturk and it is currently ACTIVE.
I also found your email account in the bounce list which could be the reason you could not reset the password via email.
I have removed the email account from the bounce list already.
Please try resetting your account password again using the "Forgot Password" link in http://www.jotform.com/login login page
Inform us if you are still not able to reset or login on your account.
Please take note that if you embedded the form using the full source code, it will require to always re-embed the form using its source code every time you update/modify/edit the form.
If there is no other reason for embedding the form to the website using its full source code, we highly recommend to use the default embed code always instead.
