Why is the form report missing fields?

I have a form with various fields (e.g., first name, middle name, last name, etc).  When I download an Excel report, entire fields (such as first name, middle name) are missing from the report, whereas other fields appear (last name).  Please help!!! Thanks!

Checking your report fields, you have left the other fields uncheck. Make sure to check all if you want to generate the entire form fields:
